Each successful grant recipient will be awarded a minimum of $10,000 to a maximum of $50,000 once approved.

All applicants must demonstrate active work and engagement with populations in zones identified by the City of Houston Health Department as at higher risk, underserved, disproportionately affected by COVID-19, or other exhibiting health-related disparities. Applicants must have current IRS 501(c)(3) designation or alternate non-profit status.

All applications must be completed electronically via the Advancing Health Equity and Outreach Education link on the HHF website beginning Friday, October 3, 2022, and submitted no later than midnight (CST) on Monday, October 24, 2022.
